What should I do when I get a medical bill?
First, check and see if this service requires a pre-notification form or not. Call Customer Service to make certain if you're not sure. Before any treatment make sure you tell the provider that you are a “CASH PAY” person. Ask your provider to give you a Ansari 1500 form or the HCFA form with all the codes for your procedure(s). If they fail to submit the form to Ansari, you are responsible for submitting the bill to us. Again, this is before treatment so that we can review the bill and negotiate the cost of service. This form is on Ansari website under Members, see "Submitting Medical Expenses" and download the MEDICAL EXPENSE SHARING REQUEST FORM for your doctor to complete or they should have a HCFA1500/UB92 form already.
What happens when a provider requires payment up front for services?
Ansari advises its members to inform providers upfront that they belong to a medical sharing organization, and that they are “cash” patients. Members should ask providers to bill them directly. However, if a provider requires a member to pay in advance for medical services, a member can submit the bill that is eligible for sharing after your IUA has been paid. Ansari will pay the bill according to your plan details. Keep in mind that your monthly contributions and other fees must be current and not delinquent for Ansari to forward any payments.
